Per Diem/Part Time Hospice MSW Social Worker

In this role, you will help guide patients and families through the hospice journey with compassion, advocacy, and thoughtful support. Responsibilities
- Perform initial psychosocial, emotional, spiritual, and bereavement assessments and assist in the development and implementation of goal-directed care plans.
- Provide care under the direction of a physician and participate in IDT conferences, observing, assessing, and sharing information regarding psychosocial, emotional, spiritual, physical, and financial conditions affecting the patient and family.
