The Opportunity
You’ll manage a large 70+ panel of Medicare patients with chronic conditions, delivering monthly clinical calls that drive real outcomes and reduce hospitalizations.
This role is built for nurses who can operate independently, manage complexity, and execute consistently at a high level.
We expect our Care Coaches to treat this role as their primary professional focus, not a side gig.
