Position Title: Case Manager, Welcoming Streets (One year contract)

Hours of Work/Schedule: 35 Hours per week (some evenings and weekends may be required)
Position Reports To: Director, Community Hub Operations
Salary Range: Annualized salary range $50,191 - $60,542, commensurate on skills and experience, plus Group Benefits, plus participation in HOOPP.
Primary Work Location: Oshawa; this position requires travel to provide services in the community and at all DCHC sites
