Care Coordinator

We're looking for an organised and proactive Care Coordinator to join our Operations team and help deliver outstanding care experiences for our clients, families and professional carers.

The Role

This is an exciting opportunity for an experienced Care Coordinator with a background in live-in care, domiciliary care, home care or community care.

You'll be responsible for coordinating care packages, managing care schedules, supporting carers, communicating with clients and families, and ensuring continuity of care across your allocated caseload.

This role would suit someone currently working as a Care Coordinator, Care Scheduler, Care Planner, Care Administrator, Care Operations Coordinator, Field Care Coordinator, Home Care Coordinator, Domiciliary Care Coordinator, Live-in Care Coordinator, Care Consultant. 

Key Responsibilities

• Coordinate live-in care packages across your allocated caseload.
• Manage client and carer rotas, schedules and shift changes.
• Coordinate new care package starts and care transitions.
• Support continuity of care through effective planning and communication.
