EMS Unlimited is preparring to launch a Community Paramedic / Mobile Integrated Health (CP/MIH) program in Garfield County, we hope to start seeing patients in early 2027. We are seeking a new team member to join soon and to be a part of the building process before it goes live. 

This is a full-time Community Paramedic role. You will work out of a dedicated CP vehicle, see enrolled patients in their homes, and work alongside their physicians to close the gaps that send people back to the emergency department. You will practice under Colorado's Community Paramedic endorsement, CP protocols, and real-time telehealth access to a supervising physician.

Who you'll see

  • Patients recently discharged who are at high risk of readmission (CHF, COPD, pneumonia, diabetes, post-surgical)
  • Frequent emergency department users and patients without a stable primary care connection
  • Patients living with chronic disease who need help managing it at home
  • Older adults at risk for falls
  • Patients with behavioral health or substance use needs who need follow-up and connection to services

What you'll do

Assessment and clinical care

  • Complete a full in-home history and physical exam, including vital signs, cardiac monitoring, and 12-lead ECG acquisition and interpretation
  • Perform point-of-care testing (i-STAT and glucose) and venipuncture/specimen collection as ordered
  • Use point-of-care ultrasound within protocol and training
  • Provide wound assessment and care, IV access, and medication administration under CP protocols and physician orders
  • Recognize decline early, escalate by telehealth consult, and arrange the right level of care (clinic, urgent care, ED, or EMS transport) when a patient needs more

Post-discharge transitional care

  • Make post-discharge home visits, typically within 24–72 hours of discharge
  • Do medication reconciliation: compare the discharge list to what is actually in the home, identify duplications, missed doses, and adherence barriers, and report discrepancies to the prescriber
  • Reinforce discharge instructions and confirm follow-up appointments are made and kept

Chronic disease management and education

  • Teach self-management for CHF (daily weights, fluid and sodium limits, symptom red flags), COPD (inhaler technique, action plans), diabetes (glucose monitoring, hypoglycemia recognition), and hypertension
  • Track patient progress against care plan goals set with the patient's physician

Safety, behavioral health, and social needs

  • Complete home safety and fall-risk assessments and recommend fixes
  • Screen for depression, substance use, food insecurity, transportation barriers, and caregiver strain
  • Connect patients to behavioral health, social services, pharmacy, home health, and community resources

Care coordination and documentation

  • Communicate findings to primary care providers, case managers, and discharge planners
  • Document every encounter and follow up on referrals until closed
  • Track enrolled patients, visits, and outcomes (ED visits, readmissions, goal progress) that the program reports to the state
  • Help build the program: CP protocols, referral workflows, and monthly quality reviews

Schedule

  • Monday through Friday, day shift - specifics of the exact schedule are still be designed
  • Travel throughout Garfield County during the shift, in a company vehicle

Pay & Benefits

  • $38.51–$45.98 an hour ($72K–$86K). They gave hires 6 months to earn the endorsement and pass the IBSC exam. That's a good local benchmark for the bracket you still need to fill.

Qualifications

Required

  • Current Colorado Paramedic certification
  • Colorado Community Paramedic endorsement completed within 2 months of hire; course and exam company-paid
  • Current CPR/BLS and ACLS
  • Minimum 2 years of paramedic-level field experience
  • Proficient in 12-lead ECG interpretation, IV access, and venipuncture
  • FEMA ICS-200 and ICS-700
  • At least 21 years old, with a valid, clean driver's license and at least 2 years of driving experience (EMS Unlimited Driver Requirements)
  • Able to work independently, manage a daily visit schedule, and communicate clearly with patients, families, and physicians
  • Comfortable with electronic charting, telehealth video visits, and basic computer work

Preferred

  • National Registry Paramedic (NRP)
  • PALS, PHTLS, AMLS
  • Critical care certification (CCP-C or FP-C) or Colorado CCT
  • Experience in home health, care coordination, behavioral health, or hospital/ED settings
  • Bilingual English/Spanish
