• Compensation (Productivity Points): $58.00 / point | $75,000-$100,000/year

Who We Are:

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model. Our mission is to dramatically improve patient access to home health care.

The PTA role is a core part of that mission: you help restore function, keep patients safe at home, and drive real outcomes — in partnership with a supervising PT.

We also build technology to support field clinicians. Our charting product reduces documentation burden, and our scheduling product (launching soon) will make routing and visit coordination easier — protecting your time.

What you will do:

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.

• Execute treatment plans with a focus on functional mobility
