Position Summary
The Transitional Aged Youth Health & Wellness Case Manager outreaches to, intakes, assesses, counsels, and coordinates care for TAY ages 18-27 years old to achieve safety, health, and well-being across multiple life domains such as housing/shelter, education and employment, behavioral health, physical health, community building, family and relationships, and much more. The Case Manager serves as the client’s main advocate and care coordinator within GLIDE’s continuum of care, community partners and resources, and other external systems of care. This position works closely and collaboratively with other TAY HWC team members to enable warm hand-offs and linkages amongst specialists, case managers, navigators, and ambassadors.
