Using a multi-disciplinary approach, you will partner with specialty clinicians to provide holistic, continuity of care for the elderly afflicted with neurocognitive disorders like dementia, Alzheimer’s disease, and other neurologic and/or psychiatric diagnoses.   What you'll do - Provide evaluation - Establish different diagnosis, i.e., Alzheimer's vs. Parkinson's vs. Vascular - Provide cognitive assessments - Provide condition-specific support therapy or when indicated, insight-based therapy - Coordinate the patient's care plan with the facility, PCP and GuideStar colleagues, and engage with the f
