The dream of Hopewell Center began in the early 1950's when a group of parents sought services for their children and founded Peter Pan Preschool. The group incorporated in 1966 and officially adopted the name Hopewell Center in May 1983.
As a private, not-for-profit agency, Hopewell has experienced rapid growth in services, personnel, and persons served. While originally focusing exclusively on persons with disabilities, Hopewell has expanded its services to families with infants and toddlers that are 'at-risk' due to developmental or socio-economic factors.
Hopewell now has an array of facility and community-based opportunities, including family services, children's services, employment services, and residential services. Today, nearly 200 caring professionals provide person-centered services to over 450 individuals a year.
Job Description
Hopewell Center is looking for a Health Advocate to support adults receiving Group Home Services. This position helps coordinate healthcare appointments, provides transportation and advocacy, communicates important healthcare information, and makes sure needed follow-up stays on track.
This is not a licensed or certified Medical Assistant position. You do not need to be a nurse or Medical Assistant to be successful in this role.
What You’ll Do
As a Health Advocate, you’ll help connect the pieces of each person’s healthcare. You will:
Schedule and coordinate medical, dental, and other healthcare appointments.
Transport and accompany individuals to appointments, helping them communicate their questions, concerns, and preferences.
Prepare appointment information and make sure provider instructions, new orders, and follow-up needs get back to the appropriate team members.
Track referrals, testing, appointments, and other healthcare follow-up to help make sure nothing gets missed.
Review and monitor electronic medication records for complete and accurate information.
Communicate with healthcare providers, families or guardians, the Residential RN, and other Hopewell team members.
Maintain accurate and confidential healthcare documentation.
Help individuals understand and participate in their healthcare decisions while supporting dignity, choice, and independence.
Administer medications when assigned and after completing required training and certification.
Provide direct support as needed and participate in the Residential Services on-call rotation as assigned.
Qualifications
What We’re Looking For
High school diploma or equivalent.
Someone who is organized, dependable, and good at follow-through.
Strong communication skills and the ability to work with individuals, healthcare providers, families or guardians, and coworkers.
Basic computer, documentation, and recordkeeping skills.
Ability to learn Hopewell procedures and complete required training, including medication administration certification.
Valid driver’s license and safe driving record.
Experience in healthcare, human services, or direct support is preferred.
Experience supporting people with intellectual and developmental disabilities is a plus.
About the Schedule
This is a full-time position with travel between Hopewell Group Homes, healthcare offices, and other community locations. Hours may vary based on appointment and service needs, and the position participates in on-call responsibilities as assigned.
