Adult Social Worker - Mental Health

Liquid Personnel is recruiting an experienced Adult Social Worker for its client's Mental Health Team based in Stockton-on-Tees. What will your responsibilities be?

As an Adult Social Worker within the Mental Health Team, you will:

• Manage a caseload of adults with severe and enduring mental health needs, undertaking assessments, support planning, reviews, and outcome-focused interventions in line with the Care Act.
• Work closely with individuals, families, carers, health professionals, and partner agencies to coordinate services, promote recovery, and support independence within the community.
• Undertake safeguarding responsibilities, mental capacity assessments, and risk management activities, ensuring appropriate support and protection plans are in place to meet identified needs.
