Experienced Adults Social Worker

Our client in Devon is seeking an experienced Adults Social Worker to join its Adults Services team based at Bideford Community Hospital. You will work within a strengths-based, person-centred service, supporting adults to maximise independence, wellbeing, and choice while delivering high-quality social care interventions.

This role offers the opportunity to work collaboratively with health partners, voluntary organisations, and multidisciplinary teams to achieve positive outcomes for adults across the community.

Key Responsibilities

• Complete Care Act assessments, reviews, and care and support planning.
• Undertake complex assessments and contribute to Continuing Healthcare (CHC) processes.
• Apply the Mental Capacity Act, including capacity assessments and Best Interest decision-making.
• Identify and promote enablement and reablement opportunities.
• Lead and complete safeguarding enquiries, including complex safeguarding investigations.
• Work effectively alongside health professionals and partner agencies.
• Undertake Court of Protection work and community deprivation of liberty applications where required.
• Maintain accurate, timely, and reflective case records.
