Social Worker - Adult Community Team

Liquid Personnel is recruiting Social Worker for its client's Adult Community Team at Manchester

What will your responsibilities be?

• Manage a caseload of adults with a range of care and support needs, promoting independence, wellbeing and choice.
• Undertake comprehensive strengths-based assessments of individuals’ care, support and social needs.
• Complete Care Act assessments and reviews, determining eligibility and identifying appropriate support arrangements.
• Develop, implement and review person-centred care and support plans in partnership with individuals, their families and carers.
• Work collaboratively with individuals to identify their strengths, goals, networks and community resources.
• Promote independence, prevention, recovery and least restrictive approaches wherever possible.
• Assess and respond appropriately to safeguarding concerns, working in accordance with local safeguarding procedures and statutory responsibilities.
• Undertake risk assessments and develop proportionate risk management plans in partnership with individuals and relevant professionals.
• Support individuals to access appropriate services, resources, benefits, housing, employment
