Job Description
Liquid Personnel is currently recruiting a Social Worker to join our client’s Community Mental Health Team in Rhondda Cynon Taf on a 9-month contract.
This is an excellent opportunity to join a well-established multidisciplinary team supporting adults with mental health needs across the Taf Ely locality, covering areas including Llantrisant, Pontypridd and Taffs Well.
Working alongside Social Workers, AMHPs, Community Psychiatric Nurses, Psychologists, Psychiatrists and Occupational Therapists, you will play a key role in delivering high-quality assessments, care planning and interventions to adults requiring mental health support.
What will your responsibilities be?
- Managing a caseload of adults with mental health needs
- Undertaking assessments, reviews and risk assessments
- Completing safeguarding work and care planning
- Conducting face-to-face visits and community-based assessments
- Working collaboratively with health colleagues and partner agencies
- Participating in duty responsibilities as required
