Social Worker - Community Team

Liquid Personnel is recruiting Adult Social Worker for its client's Community Team at Ealing.

As a Social Worker in the Adult Community Team, you will manage a varied caseload of adults with a range of social care needs. Working closely with individuals, families, health professionals, and community partners, you will promote independence, wellbeing, and informed choice while ensuring statutory responsibilities are met.
You will play a key role in supporting vulnerable adults, undertaking assessments, coordinating care, and safeguarding those at risk.

What will your responsibilities be?

• Manage a caseload of adults with varying levels of need, including complex cases depending on experience
• Complete Care Act assessments, reviews, and reassessments
• Develop and review person-centred support plans that promote independence and wellbeing
• Undertake safeguarding enquiries and manage risk effectively
• Complete Mental Capacity Assessments and apply the Mental Capacity Act where appropriate
• Support hospital discharges and work proactively to prevent unnecessary admissions
