Social Worker - Community Team

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Community team, based in Wakefield.

What will your responsibilities be? 

• Undertake comprehensive assessments of individuals’ social care needs, considering their personal circumstances, strengths, risks, and support networks.
• Develop, implement, and review person-centred care and support plans that promote independence, wellbeing, choice, and control.
• Assess eligibility for social care services in accordance with relevant legislation, policies, and local authority procedures.
• Identify and manage risks, including safeguarding concerns, self-neglect, abuse, neglect, and situations where individuals may be vulnerable.
• Coordinate and arrange appropriate care and support services, working collaboratively with individuals, families, carers, and external agencies.
