Social Worker – Disabilities

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Disabilities team, based in Powys. 

What will your responsibilities be? 

• Managing a caseload of adults with physical disabilities, sensory impairments, learning disabilities, long-term health conditions, and complex care and support needs.
• Undertaking strengths-based assessments in accordance with the Social Services and Well-being (Wales) Act 2014.
• Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence, wellbeing, and positive outcomes.
• Working collaboratively with individuals, carers, families, advocacy services, health professionals, housing providers, and partner agencies.
• Undertaking safeguarding enquiries and responding appropriately to concerns relating to abuse, neglect, exploitation, and self-neglect.
