Social Worker - Discharge to Assess Team

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Discharge to Assess team, based in Manchester. 

What will your responsibilities be? 

• Undertake timely assessments of adults with health and social care needs to support safe and effective discharge from hospital.
• Work collaboratively with hospital teams, NHS colleagues, community services, reablement teams and care providers to facilitate timely discharge.
• Apply the Discharge to Assess (D2A) model, supporting individuals to leave hospital when medically appropriate and complete longer-term assessments in the most suitable setting.
• Assess individuals' immediate care and support needs, identifying risks, strengths and available support networks.
• Develop interim care and support plans that promote safety, independence, recovery and wellbeing following discharge.
• Apply the Care Act 2014, Mental Capacity Act 200
