Social Worker - Hospital Discharge

Our client based in South London is seeking an experienced Social Worker to join their Hospital Discharge Team. This is an excellent opportunity to work within a fast-paced multidisciplinary environment, supporting adults through safe and effective hospital discharge pathways while promoting independence, wellbeing and positive outcomes.

You will work closely with health professionals, carers, families and community services to assess needs, coordinate support and ensure individuals receive the right care at the right time following hospital admission.

Key Responsibilities:

• Undertake Care Act 2014 assessments, reviews, and reassessments.
• Complete strengths-based, person-centred assessments and support planning.
• Manage a caseload of adults with learning disabilities and autism.
• Coordinate care and support services to meet assessed needs and promote independence.
• Work collaboratively with multidisciplinary teams, health partners, carers, and families.
• Advocate for service users' rights, choices, and wellbeing.
• Undertake Mental Capacity Assessments and contribute to Best Interest decisions.
• Identify and respond to safeguarding concerns in line with statutory procedures.
