Social Worker – Hospital Discharge

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Hospital Discharg team, based inWest Sussex. This is an excellent opportunity for a dedicated practitioner to support adults through safe and timely hospital discharge, helping individuals maximise their independence, wellbeing and long-term outcomes following periods of hospital admission.

The successful candidate will work within a fast-paced and supportive multidisciplinary environment, undertaking strengths-based assessments, coordinating care and support arrangements and working closely with health professionals, carers, families, community services, and voluntary sector organisations. You will play a key role in supporting individuals both within acute hospital settings and following discharge into the community, ensuring appropriate support is in place to promote recovery and independence.

This role requires a compassionate and skilled Social Worker who can respond effectively within statutory timescales, manage complex situations and support people to regain skills and confidence following illness, injury or hospital admission.
