Social Worker - Hospital Discharge

Experienced Social Workers needed for out client to support a Hospital Services and Intermediate Care Service at Salford Royal. 
These post are office based due to the nature of the business - interviews will also be face to face
Working pattern Monday to Friday 08:00 to 16:00 
3 years reference, gaps in employment history checked, any relevant quals as per JDPS

What will your responsibilities be? 

• Undertake comprehensive assessments of patients’ social care, safeguarding, housing, and support needs to facilitate safe and effective discharge from hospital.
• Develop and coordinate person-centred discharge plans in partnership with patients, families, carers, and relevant professionals.
• Assess eligibility for social care services and identify appropriate packages of care, support, and community resources.
• Coordinate referrals and access to appropriate services, including domiciliary care, reablement, supported living, residential and nursing care.
• Work collaboratively within a multidisciplinary team to ensure timely and coordinated discharge planning.
