Social Worker - Hospital Discharge

Liquid Personnel is recruiting an experienced Social Worker for its client’s Hospital Discharge Team based in Rotherham. This is a full-time agency role offering flexible hybrid working arrangements and the opportunity to support safe, person-centred discharges from hospital into the community.

What will your responsibilities be?

In this role, you will:

• Complete timely Care Act assessments and reviews to identify individuals' care and support needs following hospital discharge.
• Assess patients' strengths, needs, risks, and eligibility for social care support.
• Develop and coordinate appropriate discharge and support plans under the Discharge to Assess pathway.
• Work collaboratively with hospital teams, NHS colleagues, community services, care providers, and families.
• Arrange appropriate care packages, reablement services, short-term placements, and longer-term support where required.
• Undertake Mental Capacity Assessments and make best-interest decisions in line with the Mental Capacity Act 2005.
• Complete safeguarding referrals, enquiries, and risk management plans where concerns arise.
• Provide information, advice, and guidance to patients and carers regarding available services and support options.
