Social Worker – Hospital Social Work

Our client based in Stoke-on-Trent is seeking an experienced Social Worker to join their Hospital Social Work Team. This is an excellent opportunity for a dedicated practitioner to work within a Home First service, supporting adults to transition from temporary care arrangements into sustainable long-term support that promotes independence, wellbeing, and positive outcomes.

The successful candidate will work within a fast-paced multidisciplinary environment, undertaking assessments, care planning, and discharge-related interventions to ensure individuals receive the right support at the right time. Working closely with health professionals, care providers, carers, families, and partner agencies, you will play a key role in supporting safe transitions from hospital and temporary placements into longer-term care arrangements.

This role requires a confident Social Worker who can manage a varied caseload, work flexibly across community and office settings, and contribute to duty responsibilities whilst delivering high-quality, person-centred social work practice.<
