Social Worker – Hospital Team

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Hospital team, based in Wolverhampton. This is an excellent opportunity for a dedicated practitioner to support adults with complex care and support needs through safe, timely, and person-centred discharge planning from hospital settings.

The successful candidate will work within a fast-paced multidisciplinary environment alongside health professionals, therapists, discharge coordinators, carers, families, and community-based services. You will play a key role in ensuring individuals receive the right support at the right time, promoting independence, wellbeing, and positive outcomes whilst reducing unnecessary delays in discharge.

This role requires a confident and experienced Social Worker with strong assessment skills, safeguarding knowledge, and a sound understanding of Adult Social Care legislation. You will be responsible for managing complex cases, coordinating services, and supporting adults through periods of transition from hospital back into their communities.
