Social Worker - Learning Disabilities

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Learning Disabilities team, based in Doncaster. 

What will your responsibilities be? 

• Provide person-centred social work support to adults with learning disabilities, promoting independence, choice, rights and wellbeing.
• Complete holistic assessments of care and support needs in line with relevant legislation and local authority procedures.
• Develop, review and coordinate personalised care and support plans with individuals, families and partner agencies.
• Safeguard adults at risk of abuse or neglect, responding appropriately to safeguarding concerns and following statutory procedures.
• Work collaboratively with individuals, families, carers and multidisciplinary professionals to achieve positive outcomes.
• Support people to access appropriate health, social care, housing, education, employment and community servic
