Social Worker - Leaving Care Team

Liquid Personnel is recruiting Social Worker for its client's Leaving Care Team.

As part of a dedicated and supportive team, you will work with care-experienced young people aged 16-25, providing guidance, assessment, and pathway planning to ensure they receive the support they need to thrive. What will your responsibilities be?

• Manage a caseload of young people transitioning from care to independence.
• Undertake assessments and develop Pathway Plans in line with statutory requirements.
• Work directly with young people to identify their needs and aspirations around education, employment, training, accommodation, health, and wellbeing.
• Build positive relationships with young people, advocating on their behalf where required.
• Liaise with partner agencies and professionals to coordinate effective support and services.
• Ensure statutory visits, reviews, and recording are complete
