Social Worker - Locality Team
Liquid Personnel is recruiting Social Worker for its client's Locality Team at Herefordshire.
This is an excellent opportunity for a dedicated practitioner to work with a diverse range of adults, including older people, individuals with physical disabilities, learning disabilities, and mental health needs, delivering strengths-based and person-centred social work interventions that promote independence, wellbeing, and positive outcomes.
The successful candidate will manage a varied and complex caseload, undertaking statutory assessments, support planning, safeguarding enquiries, and risk management interventions. Working collaboratively with service users, carers, families, health professionals, and partner agencies, you will play a key role in ensuring adults receive the right support at the right time whilst maintaining choice, control, and dignity.
What will your responsibilities be?
- Managing a varied caseload involving older adults, individuals with physical disabilities, learning disabilities, and mental health needs.
- Undertaking Care Act 2014 assessments, reviews, reassessments, and support planning to identify eligible needs and promote positive outcomes.
- Developing, implementing, and reviewing person-centred Care and Support Plans
