Social Worker – Mental Health Discharge
Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Mental Health Discharge team, based in Shropshire.This is an excellent opportunity for a dedicated practitioner to support adults with complex mental health needs through safe, timely, and person-centred discharge planning.
The successful candidate will work closely with service users, carers, health professionals, housing services, and partner agencies to enable successful transitions from hospital into the community. You will play a key role in coordinating care, managing risk, promoting recovery, and ensuring individuals receive the right support to maximise independence and wellbeing following discharge.
This role requires a skilled Social Worker with experience of adult mental health services, hospital discharge pathways, and the application of the Care Act 2014, Mental Health Act 1983/2007, and Mental Capacity Act 2005.
