Social Worker - Older Adults Reviewing Team

Liquid Personnel is recruiting Social Worker for its client's Older Adults Reviewing Team at Ceredigion.
This is an excellent opportunity to join a friendly, fully staffed, and well-established team. Due to an increase in demand, the service has developed a backlog of Care and Support Plan reviews and is seeking an experienced Social Worker to provide additional capacity.

What will your responsibilities be?

• Reviewing and updating Care and Support Plans for older adults
• Undertaking assessments and reviews in line with the Social Services and Well-being (Wales) Act 2014
• Managing a caseload of older adults with varying levels of need
• Completing safeguarding enquiries where required
• Working closely with families, healthcare professionals, and partner agencies
• Promoting independence and positive outcomes for service users
• Completing all associated statutory social work duties
