Social Worker - Older People & Learning Disabilities

Liquid Personnel is recruiting an experienced Social Worker - Older People & Learning Disabilities for its client’s Older People & LD Team based in Sefton. This is a full-time agency role offering £36.79 per hour, focused on delivering high-quality reviews and reassessments as part of a targeted cost-saving project.

What will your responsibilities be?

In this role, you will:

• Review complex and high-cost care packages to establish whether the existing level and type of support remain appropriate.
• Analyse the relationship between assessed needs, identified risks, desired outcomes and the services being commissioned.
• Examine whether the current package is being fully utilised and remains effective.
• Consider whether alternative arrangements could meet the person's eligible needs appropriately.
• Identify opportunities for more effective or sustainable care arrangements without compromising the person's safety, wellbeing or statutory rights.
• Provide evidence-based recommendations regarding continuation, amendment or replacement of care packages.
