Social Worker - Transition to Adulthood Team
Liquid Personnel is recruiting Social Worker for its client's Transition to Adulthood Team at Cornwall.
This is an excellent opportunity for a dedicated practitioner to support young people with care and support needs as they move from Children's Services into Adult Social Care.
The successful candidate will work closely with young people, their families, Children's Services, Health Services, and partner agencies to ensure a smooth and planned transition into adulthood. Using a strengths-based and person-centred approach, you will undertake assessments, coordinate support, and help young people achieve positive outcomes whilst promoting independence, wellbeing, and choice.
This role requires a confident Social Worker with experience of transition work, strong assessment skills, and a sound understanding of the Care Act 2014 and Adult Social Care legislation.
What will your responsibilities be?
- Undertaking Care Act 2014 needs assessments with young people and their families to determine eligibility for Adult Social Care services.
