Job Title: Care Coordinator
Reports To: Adolescent Program Director
Location: Omaha, NE
Facility: Imagine Omaha (Adolescent outpatient)
Schedule: Choose 1 of the below
- Monday, Tuesday, Wednesday, & Friday 8:30am-5pm AND Thursday 10:30am-7pm
- Monday 10:30a-7pm AND Tuesday, Wednesday, Thursday, & Friday 8:30a-5pm
Compensation: $17-22/hour
