Case Manager Assistant

Outsourcing Staffing Advantage is seeking a Case Manager Assistant who will serve as an important point of contact for patients, families, physicians, healthcare providers, and service disciplines. This role will assist with day-to-day case coordination, communication, documentation, service orders, and follow-up to ensure patients receive the appropriate services in a timely and organized manner.

The successful candidate will work closely with an RN Case Manager and will receive comprehensive training on the company's case management processes, medical record system, service coordination procedures, and documentation requirements.

Key Responsibilities

• Assist the RN Case Manager with the day-to-day coordination and management of assigned patient cases.
• Maintain regular communication with patients and their family members regarding services and care coordination.
• Communicate professionally with physicians, medical offices, healthcare providers, and other service disciplines involved in patient care.
• Coordinate communication between patients, families, physicians, and service providers to ensure continuity of care.
• Create and process orders for services within the company's medical record system.
• Document patient communications, follow-ups, service coordination, and other case-related activities accurately and promptly.
