Care Manager Supervisor

As a Care Manager Supervisor, you will be responsible for supervising program staff.  You will provide technical guidance and training to staff related to the contract/program operations.  Care management services include:  person-centered care planning, transitions of care, residential supports, behavioral health management, case/disease/disability management and quality improvement activities. You will be responsible for monitoring compliance in accordance with client and contract deliverables as well as service/accreditation requirements as applicable.  The Illinois Tailored Care Management Program helps individuals with complex health and social needs transition from nursing facilities and Specialized Mental Health Rehabilitation Facilities (SMHRFs) to community-based living. Through person-centered care management and coordinated support services, the program empowers members to live more independently and thrive in their communities.
