Project Manager

As a Project Manager, you will be responsible for managing projects of small to medium size and complexity, following standardized Telligen project management processes. You will oversee all project related disciplines required for the successful and timely completion of the project. You will assist other project management personnel with project administration, reporting, etc. You will develop effective relationships with internal and external customers.  This is a hybrid position, requiring 3 days in office per week in our Chicago area location. The Illinois Tailored Care Management Program helps individuals with complex health and social needs transition from nursing facilities and Specialized Mental Health Rehabilitation Facilities (SMHRFs) to community-based living. Through person-centered care management and coordinated support services, the program empowers members to live more independently and thrive in their communities.
