Supervisor of Health Management

This position is responsible for supervising program staff and will provide technical guidance and training to staff related to the contract/program operations. Health management services may include: population health management, practice facilitation, medical/utilization management, quality improvement activities. The position will be responsible for monitoring compliance in accordance with client and contract deliverables as well as service/accreditation requirements as applicable.   This individual is required to be in-office two days per week- Mondays and Fridays.
