Social Work Care Manager & Patient Advocate (LSW/LCSW) - Full Time

You will carry a small, intimate private-pay caseload — deliberately kept that way — so you actually know your clients: their history, their family dynamics, what a good day looks like for them. You will build your own schedule around your visits and exercise real professional judgment, without a discharge queue, a productivity dashboard, or a supervisor assigning your day.

• $75,000–$85,000 base, plus a monthly performance bonus earned on billable hours and paid every month, never deferred to year-end
• No hospital pace. Weekdays, hybrid, with genuine flexibility in how you build your week
