We're seeking an exceptional hybrid Behavioral Health (BH) Case Manager to join our Enhanced Care Management team in Orange County, CA. Under the direction of the ECM Clinical Manager, the BH Case Manager serves as the client’s primary point of contact and works with all their providers such as doctors, specialists, pharmacists, social services providers, and others to make sure everyone is in agreement about the client’s needs and care.
