About the job

The Enrollment MA is a compassionate, self-motivated clinical professional who serves as the primary point of contact for identifying and enrolling patients into Vynca's palliative care program. In this role, you will conduct targeted outreach calls to patients, families, and healthcare organizations — educating them on palliative care and VyncaCare's services, completing structured eligibility screenings to identify patients with chronic illness and functional decline, and scheduling those who qualify for an initial visit with a nurse practitioner. Success in this position requires a strong foundation in motivational interviewing and health literacy-aware communication, as you will often be the first voice guiding patients and car
