Program Director

At WelbeHealth, we unlock the full potential of seniors by providing the support and resources they need to be happy, healthy, and as independent as possible so they can remain in their own homes. Our biggest contributions to their independence are the clinical and non-clinical services we provide at our day centers.

The Program Director, reporting to the Executive Director, oversees critical PACE (Program of All-inclusive Care for the Elderly) functions that include, but are not limited to, nutrition, social work, day center, personal care services, scheduling, and participant experience.  In addition, the Program Director is accountable for developing and maintaining a high-functioning Interdisciplinary Team (IDT).

Essential Job Duties:

• Manage critical PACE functions including, but not limited to, hiring, training, supervising, and mentoring staff, and ensuring clear and effective workflows
• Develop, plan, and facilitate an effective IDT and other participant-care related meetings, ensuring that they are organized and engaging, and that follow-up items are documented and tracked
• Effectively oversee staff within their reporting structure to drive quality care and ensure adherence to Standard Operating Procedures (SOPs) and key regulatory requirements
• Collaborate to achieve high participant outcomes w
