Social Worker - Community Wellbeing

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Community Wellbeing team, based in Herefordshire.

What will your responsibilities be? 

As part of the Community Wellbeing Team, you will manage a varied caseload and undertake a range of statutory adult social work duties, including:

• Care Act Assessments and person-centred support planning
• Mental Capacity Assessments
• Section 42 Safeguarding Enquiries
• Continuing Healthcare (CHC) work
• Section 117 aftercare responsibilities
• Multi-agency partnership working to deliver positive outcomes
• Strengths-based interventions that promote independence and wellbeing
