Social Worker - Hospital Discharge

Liquid Personnel is recruiting Social Worker for its client's Hospital Discharge Team at Manchester.

What will your responsibilities be?

• Provide timely, person-centred social work assessments to patients preparing for discharge from hospital.
• Assess patients’ care and support needs and identify appropriate discharge pathways and support arrangements.
• Work collaboratively with patients, families, carers and multidisciplinary teams to plan safe and effective discharges.
• Coordinate discharge planning with health, social care, housing, community and voluntary sector services.
• Complete Care Act assessments and develop appropriate care and support plans where required.
• Undertake Mental Capacity Act assessments and contribute to best-interest decisions where appropriate.
• Identify and respond to safeguarding concerns, ensuring appropriate action is taken to protect adults at risk.
• Support patients and families to understand their options and make informed decisions about care and support.
• Arrange and coordinate packages of care, reablement, rehabilitation, supported accommodation or other appropriate services.
