Social Worker - Mental Health

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Mental Health team, based in Manchester 

What will your responsibilities be? 

• Manage a caseload of adults experiencing a range of complex and enduring mental health difficulties, providing person-centred and strengths-based social work interventions.
• Complete comprehensive assessments of social care, mental health, safeguarding, risk and support needs.
• Develop, implement and review personalised care and support plans in partnership with service users, carers and the wider multidisciplinary team (MDT).
• Undertake Care Act assessments and reviews, including consideration of eligibility, wellbeing, prevention and least restrictive support options.
• Contribute to risk assessments and risk management plans, responding appropriately to changes in presentation, safeguarding concerns and crisis situations.
• Work
