Social Worker - Older People's Team

Liquid Personnel is recruiting Social Worker for its client's Older People's Team at Surrey.
This is an excellent opportunity to work with older adults and their families, delivering strengths-based interventions that promote independence, wellbeing and positive outcomes.

What will your responsibilities be?

• Undertake Care Act 2014 assessments, reviews, and reassessments.
• Complete strengths-based, person-centred assessments and support planning.
• Manage a caseload of adults with learning disabilities and autism.
• Coordinate care and support services to meet assessed needs and promote independence.
• Work collaboratively with multidisciplinary teams, health partners, carers, and families.
• Advocate for service users' rights, choices, and wellbeing.
• Undertake Mental Capacity Assessments and contribute to Best Interest decisions.
• Identify and respond to safeguarding concerns in line with statutory procedures.
• Assess and manage risk while promoting positive outcomes and independence.
• Maintain accurate, timely, and legally compliant case records and reports.
